Havelock North CME session: Rehabilitation Services Through Proactive

During this event, GPs and primary healthcare practitioners will hear a presentation from Sheena Lapwood, Occupational Therapist at Proactive.  Over the two hours CME session you will hear a presentation on ‘Rehabilitation Services Through Proactive’ that includes: DHB Proactive Chronic Pain Service, DHB Proactive Older Person Physiotherapy Service, ACC Pain Management, ACC Concussion Service, ACC Vocational Rehabilitation Service.

Learning outcomes:

  • Develop an awareness of services available through Proactive that GP’s can refer into or how to facilitate a referral
  • To gain an understanding of the eligibility criteria for referring into these services
  • To gain an understanding of key components of what the services provide and possible pathways post service delivery
